At the end of this retrospective study on 194 cases of breech presentation , collected between 1 January 2007 to December 31, 2007 IN ISSAKA GAZOBI maternity OF Niamey, we arrived at the following conclusions:
The frequency of breech is about 2.8% of all deliveries during this period. The average age is about 28.4 years with the extremes of 15 years and 49 years. The parity average is of 2.6 and 30% of women are primiparous. The term of the pregnancy is of 28SA to 42 SA and 81% of pregnancies are at term. Caesarean section represents the most mode of delivery with 69% and 31% delivered vaginaly. Caesarean section during labor is carried out in 51% of cases, Prophylactic caesarean section is carried out in 18% of cases. There is no significant difference between the mode of delivery of primiparous and multiparous. Indeed 72% of primiparous delivered by ceasarean section versus 68% for multiparous. The most common indication of caesarean is primiparity in 27.6% of cases followed by uterine scar in 25.4% of cases. 12 cases second twin retaining head are recorded with the rate of 20%. The overall intrapartum mortality (all deliveries combined) is of 16.5%. The APGAR index <7 at 5th minutes is found in all modes of deliveries : 18% for vaginal delivery, 13% for Caesarean section during labour and 11% for caesarean section before labor . The neonatal letality rate is of 4.9% and the main causes are respiratory distress, acute fetal suffering and fetal malformation. |
