Facteurs de risque et profil évolutif de l’asphyxie périnatale Thèse de doctorat en médecine Mariama Ayouba Zakari 78 SUMMARY: Risk factors and evolutionary profile of perinatal asphyxia. The purpose of this work was to contribute to the evaluation of risk factors and the evolutionary profile of perinatal asphyxia over a period of 07 months from 09 March to 10 October 2016. This is a prospective case-control study of neonates born at the Issaka Gazoby maternity hospital in Niamey hospitalized with neonatology. Cases (200) were selected based on the Apgar score less than seven in the fifth minute and controls for a score greater than seven in the fifth minute. Thus, out of 4123 newborns registered in the Neonatology Department 200 had presented the APN, a rate of 4.85%. The male sex had predominated with a sex ratio of 1.6. The socio-demographic factors of the mother who had influenced the AFN occurrence was lack of education and lack of employment. Referrals / evacuations (OR = 2.06), large multiparity (OR = 3.16), and unattended pregnancies (OR = 2.72) were the antepartum risk factors for AFN. Intrapartum risk factors were abnormalities of BCF with nonperceived BCF (OR = 3.58) and bradycardia (OR = 3.4), LA unspecified (OR = 2), 72), simple low pathway (OR = 4.26) and forceps (OR = 4.9). HRP (P = 0.01) and placenta previa (P = 0.01) were statistically significant pathologies detected during labor. The fetal risk factors were twins (OR = 2.6) and low birth weight (OR = 4.55). He evolution was marked by a mortality rate of 19%, 7 neonates or a rate of 3.5% were released with neurological sequelae. Key words: perinatal asphyxia, risk factors, short-term evolution, MIG |
